The Multistate Power of Attorney (POA) form may be used by a taxpayer to authorize individuals to represent the taxpayer before the tax agencies of multiple states.
This form must be submitted individually to each state where the taxpayer intends to be represented. See the instructions for more information, including the other states that accept this form.
To authorize a representative only in Colorado, use form DR 0145, Colorado Tax Information Authorization or Power of Attorney instead.
Prior authorizations will remain effective until they are expired, revoked, or withdrawn. Submit a new form only for a new authorization.
