DR 1108 - Business Tax Account Closure Form


About Form DR 1108

Use this form to notify the Department of Revenue by selecting the appropriate accounts and indicating the date of closure in order to avoid incorrect assessments of tax liability. Use this form if:

  • Your business was sold or discontinued, 
  • You are no longer liable for the tax indicated,
  • Your corporation merged into another corporation, or
  • Your business structure changed and a new Federal Employer Identification Number (FEIN) was issued.
